1 Tickets to Phish's US Fall Tour will be offered by mail order. You may use either the original form on this page or a photocopy of it to place your order. Please keep a copy of your completed order form. 


2 Print the complete address where you wish to have your tickets sent, and your phone numbers including area code in the space provided. FedEx (or international registered mail) delivery of your tickets is mandatory. You must provide a phone number including area code and a street address, or your order will be rejected; FedEx cannot deliver to PO boxes. Tickets will be shipped from August 20th through September 18th, so please be sure to provide the address and phone numbers where you will be during that time. Please take care of your tickets after they arrive; if you lose or destroy them, they cannot be replaced. 


3 Circle the type of tickets desired. Only one type of ticket may be selected for all of the shows in your order. Your order will be returned unfilled if you select "Taper only" and taper seats are sold out. Your order will be included in the lottery for regular seats if you select "Taper preferred" and taper seats are sold out. Taper seats are usually located behind the soundboard and the view from them may be obstructed. Do not order taper tickets if you do not intend to tape. 


4 Circle the price for the number of tickets desired for each show that you wish to attend; if no price is listed for 3 or 4 tickets, then the maximum number of tickets you may order for that show is 2. Send only one order form for as many shows as you wish to order; do not send a separate order form for each show requested. Each person may send only one order, and must prepare their own order. All duplicate orders will be rejected. The ticket prices listed already include a $2.50 per ticket handling fee (do not add this amount to your money orders). 


5 All orders must be paid by postal money order(s) or Western Union money order(s) ONLY, payable in U.S. dollars. Cash, personal checks, or other types of money orders will not be accepted. Make all money orders payable to PHISH. You must enclose a separate money order for each show ordered; please write which show you are ordering on each money order you send. You must also enclose one money order for the Fed Ex/registered mail fee, regardless of how many tickets you order. 


6 Keep the receipts for your money orders. Do not put the carbons from the money orders in your envelope. If any portion of your order is not filled, we will return the corresponding money order(s) to you, and you will need your receipts to get a refund. 


7 Please send your order to us in a #10 (legal size - 9.5" x 4") envelope; put your return address on the envelope and list on the bottom left front edge the date of every show you are ordering tickets for; please also write "Taper" if you are requesting tapers' tickets. Please enclose a stamped, self addressed legal size return envelope with .32 postage; we will use this envelope to return your order to you if we cannot fill any part of it. International orders, including Canada, must enclose an international postal reply coupon instead of putting US postage on the return envelope. 


8 Orders will be filled on a first-come, first-served basis by postmark. All orders must be postmarked August 13th through the 22nd; orders postmarked before August 13th or after August 22nd will not be filled. Our allotment of tickets for each show is limited, so we may not be able to fill every order that we receive; we will make every effort to have all return orders in the mail, filled or unfilled, before the tickets go on sale through regular outlets, but we cannot guarantee this. Also, please be aware that our allotment includes all levels of quality, floor seats as well as upper level and rear-of-stage seating (where applicable), the same as the range of seating that will be available when the shows go on sale through outlets. Requests for a particular seating location (other than tapers' section) cannot be honored.

Orders sent to other addresses will not be processed. No orders sent to us by Express Mail, Fed Ex, certified mail, registered mail, or "return receipt requested" will be accepted. 


9 If there are any problems with your order, please send e-mail to tickets@phish.com (best method) or call the Tickets-by-Mail problem line at (802) 865-6959 between 10 am and 5 pm Eastern time Monday through Friday after September 17th and no later than 10 days prior to the show in question. Do not call any other numbers regarding your order. Thank you.
